What you need to bring:

  • Driver licence

  • Taxi medical form (printed)

  • Full Medical History/medical summary

    • request from GP

    • or NHS app online

  • List of medications

Taxi medical forms can be downloaded from your council website.

Please bring a printed driver medical form with you on the day.

Please ensure you can access your GP summary if using the NHS app -

If we cannot see your medical history at your appointment we will not be able to complete the form.

    2. Medical summary - either printed from your GP surgery or NHS app on your phone (you do not need to print the NHS app)

    3. ID (driver licence)
